Visit the Middlesex County Public Health Department Web Site to Save Time at a H1N1 Vaccination Clinic

Director-Health Officer, David A. Papi, is encouraging residents to visit the Middlesex County Public Health Department website to print and fill out a H1N1 vaccination consent form prior to attending a clinic.

Residents can print, fill out, and bring the “Combination Consent Form” to a Middlesex County Public Health Clinic site to receive their vaccination faster.

If individuals do not have access to a computer they can visit a library in the following municipalities: Carteret, Cranbury, Dunellen, East Brunswick, Highland Park, Jamesburg, Metuchen, Milltown, Monroe, New Brunswick, North Brunswick, Old Bridge, Perth Amboy, Piscataway, Plainsboro, Sayreville, South Amboy, South Plainfield, South River, and Spotswood.

Prior to signing the consent form, residents must review the Vaccine Information Statement for either the nasal spray or the injectable vaccine.

At this time, the groups recommended to receive the 2009 H1N1 influenza vaccine include:

Pregnant women
Household contacts and caregivers for children younger than 6 months of age
Healthcare and emergency medical services personnel
All people from 6 months through 24 years of age
Persons aged 25 through 64 years who have health conditions associated with higher risk of medical complications from influenza.
Clinics will be held by appointment only from 1 to 3 p.m. Tuesdays and Fridays at the Middlesex County Public Health Clinic which is located at 596 Jersey Ave., Suite B, New Brunswick, New Jersey 08901. Residents can call 732-565-3788 between the hours of 9:00 am and 3:00 pm and press 0 to schedule an appointment for the nasal spray or injectable vaccine.

Public transportation to the clinic is available through Middlesex County Area Transit (MCAT), which offers fixed-route shuttle service via its BrunsQuik Jersey Avenue Shuttle. Riders may connect to the Jersey Avenue Shuttle from other County-run shuttles and NJ Transit buses and trains. Residents who use the shuttle must tell the driver to stop at 596 Jersey Ave.
Call 1-800-221-3520 for schedules and other information. Suggested fare is $1 or 50 cents for senior citizens and residents with disabilities.

Residents who have general questions about H1N1 or the vaccine can call the New Jersey Department of Health and Senior Services’ toll-free public information hotline from 9 a.m. to 5 p.m., Monday through Friday at 1-866-321-9571.
