Post subject: Cranbury School Immunization Requirements

Cranbury School Immunization Requirements
These requirements adhere to the State of New Jersey Department of Health and Senior Services state law stated in the New Jersey State Sanitary Code for Immunization of Pupils in Schools in Chapter 14. Please see page 2 for proper evidence for documentation for immunization records.
The following are required for pupils for 2008-2009 school year and need to be fulfilled before students enter school:
Pre-School:
DPT: 3 doses before 4th birthday
Polio: 2 doses of OPV or IPV before 4th birthday
Measles, Mumps, Rubella (MMR): 1 dose on or after the 1st birthday
Varicella: At least 1 dose on or after the 1st birthday
Hib: At least 1 dose on or after the 1st birthday
Influenza: Annual dose by December 31st yearly until age 59 months
Pneumococcal conjugate: Series of doses depending on age
Kindergarten entrance:
DPT: 4 doses with 1 on or after 4th birthday
Polio: 3 doses of OPV or IPV with 1 on or after 4th birthday
Measles: 2 doses
Mumps and Rubella: 1 dose
Hepatitis B: 3 doses
Varicella: 1 dose
If your child is less than 59 months of age upon entering Cranbury School, the Influenza and Pneumococcal conjugate vaccines are required.
Students entering 6th grade in 2008-2009:
Tdap (tetanus, diphtheria, and pertussis): Students born on or after January 1, 1997 and enrolled in grade 6, transferring into a New Jersey school from another state or country need to receive 1 booster dose.
Meningococcal: Born on or after January 1, 1997 and enrolled in grade 6 or transferring into a New Jersey school from another state or country to receive 1 dose
Documents accepted as evidence of immunization
• An official school record from any school, preschool, or childcare center
• A record from any public health department
• A certificate signed by a physician licensed to practice medicine or osteopathy or an advanced practice nurse in any jurisdiction of the U.S.
• The official record of immunization from the New Jersey
Immunization Information System
• All immunization records submitted in a language other than
English have to be accompanied by a translation sufficient to determine compliance
