Please see page 2 for proper evidence for documentation for immunization records. <br /> <br />The following are required for pupils for 2008-2009 school year and need to be fulfilled before students enter school: <br /> <br />Pre-School: <br /> <br />DPT: 3 doses before 4th birthday <br />Polio: 2 doses of OPV or IPV before 4th birthday <br />Measles, Mumps, Rubella (MMR): 1 dose on or after the 1st birthday <br />Varicella: At least 1 dose on or after the 1st birthday <br />Hib: At least 1 dose on or after the 1st birthday <br />Influenza: Annual dose by December 31st yearly until age 59 months <br />Pneumococcal conjugate: Series of doses depending on age <br /> <br /> <br />Kindergarten entrance: <br /> <br />DPT: 4 doses with 1 on or after 4th birthday <br />Polio: 3 doses of OPV or IPV with 1 on or after 4th birthday <br />Measles: 2 doses <br />Mumps and Rubella: 1 dose <br />Hepatitis B: 3 doses <br />Varicella: 1 dose <br /> <br />If your child is less than 59 months of age upon entering Cranbury School, the Influenza and Pneumococcal conjugate vaccines are required. <br /> <br />Students entering 6th grade in 2008-2009: <br /> <br />Tdap (tetanus, diphtheria, and pertussis): Students born on or after January 1, 1997 and enrolled in grade 6, transferring into a New Jersey school from another state or country need to receive 1 booster dose. <br /> <br />Meningococcal: Born on or after January 1, 1997 and enrolled in grade 6 or transferring into a New Jersey school from another state or country to receive 1 dose <br /> <br />Documents accepted as evidence of immunization <br /> <br />• An official school record from any school, preschool, or childcare center <br />• A record from any public health department <br />• A certificate signed by a physician licensed to practice medicine or osteopathy or an advanced practice nurse in any jurisdiction of the U.S. <br /> <br />• The official record of immunization from the New Jersey <br /> Immunization Information System <br /> <br />• All immunization records submitted in a language other than <br /> English have to be accompanied by a translation sufficient to determine compliance <br /> <br />For more information, go to the internet at <a href="http://www.CDC.gov" target="_blank">www.CDC.gov</a> or call the CDC at 1-800-232-4636.
